How to Request a Duplicate Plate

Steps to Request Duplicate

1. File police report (for lost plates) or bring damaged plate
2. Visit your registration LTO branch
3. Submit duplicate request form + supporting docs
4. Pay duplicate fee (~₱450)
5. Wait 4-8 weeks for new plate
6. Track via LTO Tracker

Important

You cannot operate the vehicle while waiting for the duplicate without proper documentation. Your CR + OR + duplicate application receipt may serve as temporary proof.
